About the Program
DO Transitions is designed to provide person-centered transition and habilitation services for young adults ages 18+ with developmental and/or intellectual challenges. There is no maximum age or time limit. Participants are seen and treated as young adults, with progress toward individual goals evaluated, documented, and celebrated on a regular basis..
How Does It Work?
Through person-centered goal-setting, targeted training, and community support, we promote responsibility and independence. We are establishing partnerships with local agencies and businesses to provide opportunities for volunteering, internships, and employment. The program will also serve participants in need of independent and assisted living skills. Habilitation support could be as basic as brushing teeth or as advanced as finding and scheduling transportation independently.
A Typical Day…
A typical day for DO Transitions allows for dedicated vocational training and assisted daily living/life skills activities, with staff ratios remaining small. Additionally, participants in this program spend a significant portion of each day out in the community performing hands-on activities, as we feel strongly that integration is the key to their success.
